In cases of azoospermia where the sperm cannot be retrieved from the testis, epididymal sperm aspiration (MESA) and percutaneous epididymal sperm aspiration, or PESA, are two of the few options for obtaining immature sperm cells. Both procedures involve opening the epididymal tubules and aspirating sperm, but whereas TESA involves puncturing the testicle, PESA involves inserting a needle into the epididymis. Unlike TESA, PESA has a low rate of complications such as hematomas. However, the ability to collect a large number of motile sperms using either TESA or PESA can significantly improve embryo cleavage rates, blastulation rates, and implantation rates in IVF ICSI cycles. In the five couples that Arizona Center for Fertility Studies has used fresh MESA sperm from the epididymal tubes for, three have led to live births. These results suggest that the use of MESA sperm from the epididymis can help to overcome the poor spermatogenic failure rates of NOA patients. mesa tesa
